In order to save time before your first or next appointment, some of the information Morledge Medical® would like for your file is being gathered from the forms below. Please print out and fill in the appropriate form(s) and bring them with you to your appointment.

Forms

Prescription Refill Request – call (212) 583-2830. Please allow 24 hours for a prescription refill request to be called into your pharmacy. Some prescriptions, however, will not be renewed automatically; patients on a medication for an extended period of time may require an appointment to be monitored or re-evaluated for a new written prescription.

Controlled Prescriptions cannot be called in or emailed to pharmacies. Patients taking these drugs must be evaluated in person for a controlled drug refill.

Financial Policy & Insurance Options

Billing and Payment Information

Morledge Medical® will bill your insurance provider, however, you may be responsible for other charges not covered by your insurance at the time of your appointment in addition to a co-payment. Questions regarding insurance coverage and benefits should be directed to your employer or insurance company. We accept Visa, Mastercard, Discover and American Express.

Pre-Travel Consultation Visit

The best way to have a safe & healthy trip is to properly prepare ahead of time. To gain the most benefit from a Travel Consultation, please come fully prepared to discuss your itinerary, planned activities and immunization history. We will advise you on preventive strategies and at the end of the consultation, you will be given the appropriate vaccinations and any special travel-related prescriptions needed. A Pre-Travel Health Consultation should take place 4 to 6 weeks before departure. Please bring information about your immunization history to your appointment.
